Directed at patients with advanced chronic illnesses such as: Chronic heart failure, coronary heart disease, chronic respiratory disease, chronic kidney disease, chronic liver disease, chronic neurological disease, diabetes mellitus, solid or active haematologic neoplasm not susceptible to curative treatment, chronic osteoarticular disease.
The treatments are performed on an outpatient basis in an external consultation, in a day hospital or with admission if necessary due to decompensation that cannot be managed through the outpatient service. Other s pharmacologic therapeutic reconciliation, rehabilitation, telephone access to health education and patient training in self-care.
